This Acknowledgment of the Terms and Conditions of Program Participation is an essential legal document relating to my consideration for and participation in the Assoc of Miltary Spouse Entrepreneur™ Award

I represent, warrant, and affirm as follows:

1. I am legally married to an active duty Reserve or Guard member of the U.S. military.

2. I possess a dependent ID card issued by the Department of Defense.

3. I have an established business with an EIN through the IRS

4. I am at least 18 years of age.
